We have several assessments available that may help us formulate a more comprehensive approach to treatment based on the unique needs of your child.

Assessments available through Ensemble Therapy:

Parenting Stress Index — Measures the extent of stress in the parent-child relationship and can help identify contributing factors to dysfunctions in parenting or problematic child behaviors.

Child Behavior Checklist + Youth Self Report + Teacher Report Form — Measures emotional and behavioral problems. These three assessments gather the same information from three different sources to get an accurate picture. Parents complete the Child Behavior Checklist, 11-18 year olds complete the Youth Self Report, and then teachers complete the Teacher Report Form.

Attention Deficit Disorder Evaluation Scale Fourth Edition — Parent or teacher assessment, measures inattentive and hyperactive-impulsive type.

Revised Children’s Manifest Anxiety Scale — Measures the level and nature of anxiety as experienced by children today.

Children’s Depression Inventory — Assesses cognitive, affective and behavioral signs of depression in children and adolescents.

**Please note that all assessments are available for an additional fee. Should you be interested in adding any of these to your child’s treatment plan, inquire with your therapist about which would be a good fit for your family’s needs. Many of these assessments may be completed on your own time via a questionnaire that we email to you. Others may be done with your child, and we’re happy to share more about the differences of each.
